What order should I do this in?

1. Answer one question: do you have student debt? It routes everything. With loans you are looking at NC LRP or PCPI, up to $100,000 non-taxable. Without, you are looking at HNSB, up to $100,000 taxable — and the physician loan's main advantage stops applying to you. The no-debt track.

2. Find out who will own your practice. PCPI requires physician ownership, not a hospital or health system. That is on your offer letter. The clause.

3. Look at the market where you are actually going. North Carolina's big metros fell and its incentive counties rose. The data.

4. Then pick the loan. Physician loan or conventional.

Before 25 March 2022 NC LRP required providers to access NHSC first and sites to be NHSC-certified; a FY2022 temporary budget increase removed that, and ORH "anticipates that these temporary revisions will be removed once the temporary funds have been exhausted." HNSB's minimum HPSA score was temporarily cut from 16 to 10. PCPI launched on one-time FY2024 funding. And: "Guidelines are subject to change at any time, at the discretion of ORH. Incentive programs are contingent upon funding."

The excise tax

$1 per $500 = 0.2%, and §105-228.30(a): "The transferor must pay the tax." On $400,000 that is $800, charged to the seller.

Against Philadelphia's 4.578% with grantor and grantee jointly and severally liable: $18,312 with the buyer exposed. A $17,512 difference, and the liability runs the other way. ★ Eight exemptions, including a lease for a term of years — the opposite of Pennsylvania.

What the physician loan does

  • Up to 100% financing with no PMI; five-percent-down options; to $2M.
  • Student debt on your documented income-driven payment, not 1% of balance (Fannie Mae B3-6-05) or 0.5% (HUD 4000.1).
  • Closing up to 150 days before your start date on a signed contract, against the 90 agency rules allow.
  • Eligible degrees: MD, DO, DDS, DMD, DPM, OD, PharmD, DVM, CRNA, plus residents and fellows.
  • Asset depletion as supplemental income only, at a 3% rate of return, assets in a US account.

★ And the honest caveat: if you have no student debt, the second bullet is worth nothing to you, and conventional deserves a real comparison. Both, priced.

Loan limits

$832,750 in all 100 counties, no high-cost exception anywhere, even on the Outer Banks. Kill Devil Hills at $599,233 still leaves about $233,517 of room. Detail.

Every figure carries the date 2026-10-06. The excise tax comes from the North Carolina General Statutes read directly on ncleg.gov. The incentive figures come from the Office of Rural Health's own page. Loan limits come from the FHFA's 2026 county file, all 100 counties checked. Market data is Zillow Research's metro series to 31 August 2026.

What should a physician know before buying in North Carolina?

Four things. Whether you have student debt decides which incentive programme you can use and how much the physician loan helps you. Whether your practice is physician-owned decides whether PCPI's $100,000 is available. The big metros fell over the past year while the incentive counties rose fastest. And the excise tax is only 0.2% and the statute charges it to the seller.

How much does North Carolina pay physicians?

Up to $100,000 non-taxable under the NC Loan Repayment Program or the Primary Care Physicians Initiative, for an up to four-year service commitment, and up to $100,000 taxable under the High Needs Service Bonus for providers without educational loan debt. Nurse practitioners, nurse midwives, physician assistants and dental hygienists receive up to $60,000. Verified against ORH 2026-10-06.

What does a North Carolina physician loan offer?

Up to 100% financing with no private mortgage insurance, five-percent-down options, loan amounts to $2M, student debt counted on your documented income-driven payment rather than 1% of the balance, and closing up to 150 days before your start date on a signed employment contract. Eligible degrees include MD, DO, DDS, DMD, DPM, OD, PharmD, DVM and CRNA, plus residents and fellows.

Is North Carolina a good state for a physician to buy in?

It depends sharply on where. The incentive counties were the fastest-appreciating markets in the state with homes between $116,000 and $153,000, while Raleigh, Durham, Charlotte and Asheville all lost value over the year to 31 August 2026. Transfer costs are low at 0.2% and statutorily the seller's, and the $832,750 conforming limit applies in all 100 counties.
