Physician Loan or Conventional? The North Carolina Answer

Most physician-loan pages argue in one direction. North Carolina produces two situations where the honest answer is the other product.

★★ Case one: you have no student debt

This is the North Carolina case, because North Carolina has a programme built for it. HNSB pays providers without educational loan debt up to $100,000 taxable for the same up-to-four-year commitment.

If that is you, then the physician loan's biggest advantage — qualifying you on your documented payment instead of 1% of a balance — is worth exactly nothing, because there is no balance. What remains is the no-PMI structure at up to 100% financing and the 150-day window.

Those are real, and they still may not beat a conventional loan with a reasonable deposit. Get both priced. We would rather lose the product sale than put you in the wrong structure. HNSB in full.

★ Case two: you are buying in a market that fell

Raleigh fell 1.8%, Durham 2.2%, Asheville 3.3% and Charlotte 0.5% over the year to 31 August 2026. At 100% financing you start with $0 of equity in an asset that has been moving down.

The question worth asking is not which loan but how much to borrow. A five-percent-down option exists under the physician programme, and putting a deposit in is a different risk position from putting none in. If you expect to move within a few years, that gap matters more than the product name.

Where the physician loan clearly wins

When you carry a residency-era balance. On a $300,000 balance, agency underwriting may count $3,000 a month under the 1% rule while your documented income-driven payment is a fraction of that. Against a first-year attending income that phantom obligation is the difference between approval and decline.

It also wins on timing. 150 days against 90 covers a spring purchase for a July start, which is how most North Carolina academic hiring runs. The debt treatment and the timing.

Where FHA fits

HUD Handbook 4000.1 caps purchase loan-to-value at 96.5% of adjusted value, the familiar 3.5% minimum down payment, and sets credit tiers: at or above 580 you are eligible for maximum financing, while 500 to 579 is capped at 90% LTV.

FHA carries its own mortgage insurance. In North Carolina's cheap eastern markets it is a genuine option for a simpler file, so it deserves pricing rather than dismissal.

Where all three are identical

None of them finance the excise tax, recording fees, prepaid items or reserves. The good news in North Carolina is that the excise tax is 0.2% and §105-228.30(a) charges it to the transferor, so the identical column is small and mostly the seller's. The statute.

So which should I take?

With substantial student debt, a start date ahead of you, or a small deposit, the physician programme is usually the better structure. With no student debt, or a large deposit, or a purchase in a declining Triangle market where you want some equity from day one, conventional deserves a direct comparison and will sometimes win.

No. The physician programme offers up to 100% financing with no private mortgage insurance. Conventional financing generally requires mortgage insurance above 80% loan-to-value, and FHA carries its own mortgage insurance premiums. It is a lender portfolio programme rather than agency financing.

Will any loan cover the North Carolina excise tax?

No, it is cash due at closing under every loan programme. The good news is that North Carolina's rate is 0.2% and N.C.G.S. §105-228.30(a) provides that the transferor must pay it, so the statute charges it to the seller. Recording fees, prepaid items and reserves remain the buyer's cash requirement.
